Abstract
:
Objective: To find out the incidence of thyroid cancer in surgically treated nodular thyroid disease and to evaluate the characteristics of thyroid cancer patients to regards their demographic distributions, histological variants, mode of presentation and the modalities of diagnosis and treatment. Methods: We retrospectively examined medical records of two hundred and forty-one patients underwent thyroidec-tomy for nodular thyroid disease at King Khalid National Guard Hospital between 1996-2001. The incidence of malignancy in these patients group was evaluated and the demographic data, clinical presentation, diagnostic workup, treatment modalities and prognosis indicators were evaluated in the study. Results: Patients with thyroid cancer represent 31% of thyroidectomy cases with female to male ratio 6:1. The average age at diagnosis is 40±13 years. Thyroid swelling was the most common presentation in 73.3% of the patients. Papillary carcinoma was the most common histological variant. Ultrasonography and FNAB were the most common utilized investigation. The majority patients with thyroid cancer underwent near-total thyroidectomy and around 55% of patients had functional and upper mediastinal neck dissection with thyroidectomy. 90% of the patients with papillary cancer received radioactive iodine treatment (I131). Conclusion: Thyroid malignancy is common in Saudi Arabia and represents the second most common malignancy in Saudi female because of the higher incidence of thyroid cancer in Saudi Arabia and the severity of the local disease at the time of presentation. We recommended that thyroid cancer patients be treated in specialized center by a team of endocrinologists, pathologists, experienced thyroid surgeons and nuclear medicine consultants to achieve an optimum care and excellent prognosis
